Job description

This locum tenens Nurse Practitioner opportunity offers a flexible hybrid model combining telehealth and in-home health assessments. The role focuses on preventive care, annual risk assessments, and chronic condition reviews across adult and pediatric populations.

Schedule
Monday–Friday
8:00am–5:00pm (1-hour lunch)
Full-time or part-time options available
No call

Clinical Details
Hybrid model: approximately 70% telehealth and 30% in-home visits
All ages patient population
10–14 patients per day depending on visit type
Home visits concentrated within assigned county
Assessment-based role with no prescribing required

Responsibilities
Conduct annual wellness visits and risk assessments
Perform chronic condition reviews
Complete documentation and coordinate care with PCPs
Escalate urgent findings appropriately

Additional Details
Mileage reimbursed for home visits
Schedules released 2–3 weeks in advance
Paid orientation and onboarding modules

Requirements
Board Certified Nurse Practitioner
Active Oregon license required
Comfortable with telehealth platforms and home health visits
