Telehealth + Home Health NP needed in Bowling green, KY

Job description

Location: Kentucky
Position: Nurse Practitioner (NP)
Start Date: April 1 or ASAP upon credentialing
Contract Length: Month-to-month (option to extend)

📅 Assignment Details
Schedule: Monday–Friday
Hours: 8:00 AM – 5:00 PM
Lunch: 1-hour break included
Schedule Release: 2–3 weeks in advance
💻 Practice Setting
Model: 70% Telehealth / 30% Home Health
Travel Radius: Within 30 miles of provider’s home address
Equipment: Provided and shipped directly to provider
🩺 Work Details
Patient Population: Pediatrics + Adults (must be comfortable with both)

Daily Volume:
Telehealth: 12–14 patients/day
In-home only: 6–7 patients/day
Mixed schedule: ~10 patients/day

Responsibilities Include:
Annual risk assessments
Chronic condition reviews
Escalating urgent findings to PCP

Role Type:
Assessment-only (no prescribing or treatment required)

Paid for full day regardless of patient volume
Must enter availability into Salesforce to qualify for full-day pay
1–3 hours of paid orientation
Ongoing education modules completed during downtime

Active Kentucky license required
Must be enrolled/credentialed in Medicaid
Comfortable seeing pediatric + adult patients
Telehealth + home health experience preferred
