Hybrid FNP Opportunity – Telehealth + Home Visits in Kentucky

Month to Month with the option to extend

Position Highlights:
Full-time only (40 hours/week) Monday–Friday
8 am-5 pm with a 1-hour lunch
One-hour lunch break included
Equipment will be mailed out and provided by the client
70% telehealth, 30% Home health, will schedule home health visits within a 30-mile radius of the provided address
MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS
Schedules are released 2–3 weeks in advance, so the NP can get familiar with the patients.
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
1–3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
Must be enrolled/Credentialed in Medicard (please highlight in submission and confirm with provider)

Role Overview:
For telehealth, the NP should expect 12-14 per day on their schedule. If only doing in-home visits, they should expect 6-7. If it is a mix, it usually falls around 10
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP


Must have an active KY Medical license
