Telehealth Family Nurse Practitioner Locums – Kentucky

Job description

This locum tenens role is ideal for an FNP looking for a flexible, mostly telehealth position with a mix of virtual care and occasional home health visits.

Schedule
Monday–Friday
8a–5p
40 hours/week
1-hour lunch break

Assignment Details
Start ASAP (as soon as credentialed)
Month-to-month with option to extend

Clinical Setting
Telehealth + home health hybrid
EMR: TBD

Scope of Work
Conduct annual risk assessments and chronic condition reviews
Manage adult and geriatric patients
Assessment-focused role (no prescribing required)
Escalate findings to patient’s PCP

Patient Volume
~12–14 patients/day (telehealth)
If doing home visits only: ~6–7/day
If mixed: ~10/day

Coverage Model
~70% telehealth
~30% home visits within a 30-mile radius

Additional Details
Equipment provided by client
Schedules released 2–3 weeks in advance
Paid for full day regardless of patient volume (must be available)
Orientation and ongoing modules required

Requirements
FNP
Active Kentucky license
Must be enrolled/credentialed in Medicare
