✔️ Active NY State Nurse Practitioner license (required)

✔️ Perform health assessments and exams in patients’ homes
✔️ Diagnose and manage acute/chronic conditions
✔️ Develop individualized care plans

✔️ Order and interpret diagnostic tests
✔️ Educate patients and families on health maintenance
✔️ Coordinate care with physicians and healthcare team
✔️ Maintain accurate documentation of patient encounters
