Hybrid FNP Opportunity – Telehealth + Home Visits in Oregon

Job description

Location: Lane, Multnomah, or Washington County (please specify preference)
Schedule: Full-time, Mon–Fri, 8a–5p; will consider part time as well

Overview:
Hybrid role with 70% telehealth and 30% in-home visits. Focus on annual assessments and chronic condition reviews for pediatric and adult patients. No prescribing required.

Contract Length: 3 months with the option to extend

Position Highlights:
Full-time (40 hours/week) Monday–Friday or Part-time available (20 hours)
8 am-5 pm with a 1-hour lunch
Must be comfortable and able to see Pediatrics and Adults
Schedules are released 2–3 weeks in advance
Open to full-time or part-time providers
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
Must be enrolled/Credentialed in Medicare

Role Overview:
For telehealth, the NP should expect 12-14 per day on their schedule. If only doing in-home visits, they should expect 6-7. If it is a mix, it usually falls around 10
Home Health visits will be in the Milwaukee area
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP
