Clinical Details
Home health setting
Average 6–9 patient visits per day
Adult and geriatric patient population
In-home health evaluations for Medicare Advantage, Medicaid, and commercial patients

Scope of Work
Conduct comprehensive in-home health assessments
Perform medication reconciliation and medical history reviews
Complete documentation and care plans
No prescribing, ordering labs, or altering treatment plans required

Requirements
NP or PA certification
Active Pennsylvania license
Board certification required
