Hybrid FNP Opportunity – Telehealth + Home Visits in Milwaukee, WI

Month to Month with the option to extend

Position Highlights:
-Full-time only (40 hours/week) Monday–Friday
-8 am-5 pm with a 1-hour lunch
-One-hour lunch break included
-Equipment will be mailed out and provided by the client
-70% telehealth, 30% Home health, will schedule home health visits within a 30-mile radius of the provided address
-MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS
-Schedules are released 2–3 weeks in advance, so the NP can get familiar with the patients.
-Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
-1–3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
-Must be enrolled/Credentialed in Medicare

Role Overview:
-For telehealth, the NP should expect 12-14 per day on their schedule. If only doing in-home visits, they should expect 6-7. If it is a mix, it usually falls around 10
-Home Health visits will be in the Milwaukee area
-Perform annual risk assessments and chronic condition reviews
-Assessment-only role — no treatment or prescribing required
-Escalate urgent findings to the patient’s PCP
-Must have an active WI Medical license
