✔️ Associate or Bachelor’s Degree in Nursing
✔️ Active NYS RN license
✔️ Minimum 6 months clinical experience
✔️ Proficient with HCHB Clinical Software
✔️ Valid driver’s license and reliable transportation
✔️ Strong communication & critical thinking

✅ Key Responsibilities
✔️ Perform comprehensive home care assessments & develop patient care plans
✔️ Provide skilled nursing interventions including wound care, injections, and medication management
✔️ Document all care and patient responses accurately in HCHB
✔️ Manage caseload and coordinate care with patients, families, physicians, and interdisciplinary teams
✔️ Complete timely visit documentation and follow-ups
✔️ Conduct discharge planning and support infection control standards
