✅ Key Responsibilities
✔️ Administer medications and prescribed treatments
✔️ Monitor and record patient vitals and health conditions
✔️ Support individualized care plans with healthcare team
✔️ Assist residents with daily living activities (ADLs)
✔️ Maintain accurate and timely medical documentation
✔️ Follow infection control and safety protocols
✔️ Respond quickly to patient emergencies

✅ Qualifications
✔️ Completion of accredited LPN program
✔️ Active NY State LPN license required
✔️ Minimum 1 year long-term care or hospital experience
✔️ Proof of Hepatitis B immunity required
✔️ Strong communication and teamwork skills
✔️ Ability to work rotating weekends

• 1 Week of Paid Vacation based on accruals after 3 months of employment
• 5 Sick Days (40 Hours) subject to the provisions of NYS Paid Sick Leave Act
