Position Overview
We are seeking a Licensed Practical Nurse (LPN) to join a union position at a mission-driven Federally Qualified Health Center in Brooklyn, NY. This role focuses on primary care support, patient education, and coordinated care delivery in underserved communities.

Compensation & Benefits

Sign-On Bonus (Based on Experience)
✔ 0–5 years: $5,000
✔ 5–10 years: $10,000
✔ 10+ years: $15,000

Qualifications
✔ Certificate from an accredited Practical Nurse program
✔ Active New York State LPN license and registration
✔ 1–2 years ambulatory experience preferred
✔ Venipuncture, medication administration, and immunization skills
✔ Basic computer skills including typing
✔ Strong communication, organizational, and interpersonal skills
✔ Bilingual Spanish preferred

Key Responsibilities
✔ Administer medications, treatments, and wound care under RN supervision
✔ Monitor and document patient conditions and vital signs
✔ Assist in care plan implementation with RN and providers
✔ Educate patients and families on medications and recovery
✔ Maintain accurate clinical documentation and medication records
✔ Follow infection control and safety protocols
✔ Provide patient support and advocacy
