Home Health

Conduct pre-scheduled in-home health evaluations

6–9 visits per day

Patient population includes Medicare Advantage, Medicaid, and select commercial plan members

Review medical history & medications and complete physical evaluation

No prescribing, Labs ( no follow up), no treatment changes

Pre-populated, patient-specific documentation provided


Travel, Lodging and Daily Gas Stipend Available

Active AZ state license

Credentialing: 1–3 weeks
