Family/Geriatric Physician needed part time weekly - flexible hours

Job description

We offer remote senior care services including phone/app checkins, concierge services and remote patient monitoring in line with Medicare CCM/RPM programs. We are looking for a Physician to start part time with us to help run our CCM/RPM programs.
You will have a team of RNs and CNAs reporting to you and escalating Chronic care issues to you. You will actively collaborate with patients' primary care / specialist physicians to ensure we ensure team based care for our senior clients.
Requirements
1) Pennsylvania Licensed MD/DO
2) Familiarity with using EMR systems - preferably E-clinicalworks.
3) Passion for helping seniors
