In-Home Health Risk Assessments - Yakima Washington area
Start Date: ASAP
Scheduling and Flexibility: Monday-Sunday 7a-7p
*Choose your availability
*Most providers work anywhere from 4 to 8 shifts per month
*Each visit typically takes about 45 to 60 minutes
*Set number of visits scheduled for each shift in your local area
Conducting in-home health risk assessments for Medicare Advantage members. These visits are scheduled at the patient’s home — it’s not urgent care or ongoing treatment, but a one-time, preventive visit meant to provide a full health picture back to the insurance plan.
What You'll Be Doing:
Reviewing the patient’s medical history and current medications
Performing a basic physical exam (vitals, heart/lungs, mobility checks, etc.)
Asking about lifestyle, home safety, and any social/behavioral concerns
Documenting diagnoses using ICD-10 codes (training is provided)
Educating patients on preventive screenings and answering their questions
You will not be diagnosing or prescribing — this is strictly an evaluation and documentation visit.
