Practice Setting
Home Health
Conduct pre-scheduled in-home health evaluations
6–9 visits per day
Patient population includes Medicare Advantage, Medicaid, and select commercial plan members
Review medical history & medications and complete physical evaluation
No prescribing, no ordering labs, no treatment changes
Pre-populated, patient-specific documentation provided
Travel - Airfare, hotel, and rental car available (based on eligibility)
Requirements
Active WV state license
Local Preferred
Credentialing: 1–3 weeks
