A Plus Quality Home Care is seeking an experienced and dependable PRN Clinical Assessor to join our growing team.

This position is ideal for a licensed professional who is organized, compassionate, and comfortable working independently in the field.

Position Overview:
The PRN Clinical Assessor will travel to client residences to complete quarterly and annual assessments, evaluate care needs, and ensure compliance with state requirements. This role plays a vital part in maintaining the quality and integrity of services provided to our patients.

Responsibilities Include:
*Conducting quarterly and annual in-home client assessments
*Evaluating patient care plans and updating documentation as needed
*Assessing caregiver competency related to ADLs (Activities of Daily Living)
*Assisting with caregiver skill evaluations and state-required training
*Ensuring all documentation meets regulatory standards
*Communicating findings clearly with office staff and caregivers

Qualifications:
*Active professional license (RN preferred; LPN considered based on experience)
*Home care experience required
*Strong understanding of ADLs and caregiver skill competency
*Excellent documentation and communication skills
*Reliable transportation and willingness to travel locally
*Professional, personable, and compassionate demeanor
*Ability to work independently with minimal supervision
