Job description

Facility serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a Facility service area. While all Facility participants must be certified to need nursing home care to enroll in Facility , only about 7 percent of Facility participants nationally reside in a nursing home. If a facility enrollee needs nursing home care, the facility program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, facility is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

Job Description
General Responsibilities will include:
-Perform comprehensive medical assessments
-Develop and implement individualized care plans
-Provide acute illness evaluation and treatment
-Participate actively in discharge planning
-Conduct home visits as clinically indicated
-Coordinate care across the full continuum of services offered by facility
-Assist in the development and review of clinical policies and procedures
-Participate in provider on-call rotation

Requirements
-Board Certified Nurse Practitioner (ANCC or AANP) – [REQUIRED]
-Active Massachusetts NP license – [REQUIRED]
-DEA Registration – [REQUIRED]
-CDS Certification – [REQUIRED]
-BLS Certification – [REQUIRED]
-Experience with frail or elderly population – [REQUIRED]
-Ability to travel within service area – [REQUIRED]
