Job description
📅 Assignment Details
Schedule: Flexible (Monday – Sunday)
Hours: 7:00 AM – 7:00 PM
❌ No nights or weekends required

🩺 Work Details
Patient Volume: 6–9 visits/day
Responsibilities Include:
Conducting pre-scheduled in-home health evaluations
Reviewing medication and medical history
Performing physical assessments
Completing pre-populated documentation

📌 No prescribing, no labs, no treatment changes

Medicare/Medicaid health evaluations
Chronic condition reviews

Provider Type: PA
Board Certification: Required
Education: Master’s required
License: Active state license required
