Telehealth + Home Health NP needed in OR!

Job description

📅 Assignment Details
Schedule: Monday – Friday (flexible; weekends/nights optional)
Hours: Full-time (40 hrs/week) or part-time options available
Schedule Release: 2–3 weeks in advance
Includes 1-hour lunch
🏥 Practice Setting
Setting: Telehealth (70%) + Home Health (30%)
Travel Radius: Within 30 miles of provider’s home
🩺 Work Details

Patient Volume:

Telehealth: 12–14 patients/day
In-home only: 6–7 visits/day
Mixed schedule: ~10 patients/day

Responsibilities Include:

Annual risk assessments
Chronic condition reviews
Documentation and care coordination
Escalating urgent findings to PCP

📌 No prescribing or treatment required

Preventative care and risk assessment
Adult and pediatric patient population

✅ Requirements
Provider Type: NP
License: Active Oregon license REQUIRED
Must be comfortable seeing pediatrics + adults
Medicaid enrollment required (must be credentialed and confirmed at submission)
