Job Description
-Provide primary medical care in collaboration with physicians
-Perform initial, semi-annual, annual, and as-needed participant assessments
-Develop and integrate individualized care plans within the interdisciplinary team (IDT) framework
-Evaluate and treat acute illnesses while managing chronic conditions
-Deliver preventive care including screenings, immunizations, and ongoing monitoring
-Prescribe medications and therapies appropriately
-Refer participants to specialists when necessary
-Participate in discharge planning and transitions of care
-Educate patients, families, and caregivers on treatment plans and health management
-Serve as an active member of the interdisciplinary team (IDT)
-Assist with development of clinical policies, procedures, and standards of care
-Participate in staff education and training initiatives
-Maintain accurate, timely, and compliant clinical documentation
-Participate in on-call rotation schedules
-Ensure compliance with all regulatory and organizational policies
