NURSE PRACTITIONER – GERIATRIC PACE CLINIC

Start Date: ASAP – Ongoing
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Setting: PACE Clinic
EMR: EPIC
Patient Population: Geriatric / Frail & Elderly Patients

REQUIREMENTS

✓ Board Certified – ANCC or AANP
✓ Active North Carolina License required
✓ BLS required
✓ CDS/Controlled Substance Certification required
✓ DEA Registration required
✓ Minimum 1 year of experience working with frail or elderly patients
✓ Experienced providers only – No New Grads
✓ Geriatric experience preferred
✓ Must be able to practice autonomously
✓ Medical Director available on-site for support
✓ Credentialing timeframe may take up to 45 days

ADDITIONAL DETAILS

✓ Full-time daytime schedule
✓ Monday–Friday, 8:00 AM–5:00 PM
✓ EPIC documentation
✓ Geriatric-focused patient population
✓ Provider may need to obtain the state license, but an active NC license is required for submission
✓ Local providers preferred; mileage reimbursement only
