Opportunity:
Advanced Practitioners - Physician Assistant - Geriatrics

Job Details:

  • Full-time Nurse Practitioner or Physician Assistant position supporting the home-based REACH (Resource for Elderly and Care at Home) Program
  • Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)
  • Deliver compassionate geriatric care in patients' homes and assisted living facilities in the greater Manchester, CT area
  • Work within a multi-disciplinary care team focused on Geriatric Medicine in the home setting
  • Monday through Friday schedule with no weekend requirements
  • Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
  • Develop plans for further evaluation of patient health needs, including referrals to specialists and other health team members
  • Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
  • Unrestricted State of Connecticut Medical License required (or eligible)
  • Minimum of two years of post-graduate experience as a Nurse Practitioner or Physician Assistant in Geriatric Medicine or a related field required
