Opportunity for a Locum Physician specializing in Geriatric Medicine to serve in a Medical Director role within a PACE (Program of All-Inclusive Care for the Elderly) model. The position focuses on providing comprehensive, community-based care for seniors with chronic healthcare needs, emphasizing independence, coordinated care, and improved clinical outcomes.
๐๐๐ฒ ๐๐๐ฌ๐ฉ๐จ๐ง๐ฌ๐ข๐๐ข๐ฅ๐ข๐ญ๐ข๐๐ฌ:
- Provide medical direction and supervision for participant care.
- Conduct history and physical exams for new participants and reassessments.
- Develop treatment plans and deliver medically necessary services across care settings.
- Collaborate with interdisciplinary teams on care planning and decision-making.
- Oversee Quality Assurance Performance Improvement (QAPI) program activities.
- Supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
- Participate in intake assessments, family conferences, and ongoing care coordination.
- Educate patients and families regarding chronic conditions and preventive health.
- Manage frail elderly populations with chronic disease care needs.

๐๐๐ช๐ฎ๐ข๐ซ๐๐ฆ๐๐ง๐ญ๐ฌ:
- Board Certification in Internal Medicine or Family Practice (Required)
- Board Certification in Geriatrics (Preferred)
- Active Massachusetts physician license (Required)
- DEA registration (Required)
- Minimum 1 year experience with frail or elderly population (Required)
- Previous supervisory experience (Required)
- Recent NPDB self-query (Required)
- Medical Director Leadership Role
- PACE Model of Care
- Full-Time Weekday Schedule
