The facility features a 99-bed department with strong interdisciplinary support, including dedicated APP coverage, and hospitalists covering night call to ensure a manageable and collaborative work environment.

Physician - Internal Medicine - Geriatric Medicine

Job Details:

• Provide locum tenens coverage for the Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit with 12-14 patient encounters per shift
• Schedule is three days per week (preferably Tuesday, Wednesday, and Thursday), 10-hour shifts from 7a-5p with no call required
• Dedicated APP support provided on campus
• Specific coverage dates needed: Sept 15-17, Sept 29-Oct 1, Oct 13-15, Nov 3-5, Nov 10-12, Nov 17-19, Nov 23-25 (Monday-Wednesday due to holiday)
• Board-certified or board-eligible Internal Medicine or Family Medicine physician required
• Active Montana license or IMLC required to bid
• ACLS and BLS certifications required
• Estimated credentialing timeframe of 120 days; temporary/emergency privileges available
• Review and approve admissions to the SNF, including completing comprehensive history and physical examinations for all new admissions
• Conduct required physician compliance and recertification visits in accordance with CMS and regulatory requirements for both SNF and LTC residents
• Provide ongoing primary medical care for long-term care residents, including management of chronic medical conditions and preventative care
• Develop, review, and update individualized care plans in collaboration with nursing, rehabilitation services, case management, and social work
• Manage acute changes in condition, evaluate new symptoms, and provide treatment to reduce unnecessary emergency department transfers and hospital readmissions
• Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety
• Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate
• Coordinate transitions of care between the hospital, skilled nursing, long-term care, specialty services, and community providers
• Conduct routine medical evaluations and chronic disease management for long-term care residents
• Perform annual wellness and preventive care assessments
• Provide oversight of palliative and comfort-focused care plans when appropriate
• Collaborate with families and healthcare decision-makers regarding changes in condition, treatment plans, and advance care planning
• Complete certification and documentation required to maintain residents' long-term care plans and regulatory compliance
