Locums Eureka, CA Hematology/Oncology PA or NP *Please read additional details regarding opportunity for an earlier start* CLEAN FILES PREFERRED. *We are willing to look at candidates that can give 2 weeks a month or more. We will need 2 full-time providers to fill the role

SCHEDULE • Coverage Type: Outpatient • Schedule Type: Clinic No call • Dates of Need: Full time need • Shift Times: 8am-12pm, 1pm-5pm • Lunch TBD 1 hour lunch unpaid • Provider Min # Shifts Required: 2- 4 weeks per month • Guaranteed Hours per Day: 8 hours per day • Hours Worked for OT to apply: 9 hours per day (All OT should be preapproved by your clinic manager (Marci D). Please seek preapproval via email with Marci. If you have any questions, please escalate to the PSJH admin team) • Rates to be Filled Out on Bid: Clinic: Regular, Orientation, Overtime, Holiday

CREDENTIALING • Current CV (must be uploaded to bid) • License: Active only • DEA Required: Yes • Boards: BC or truly BE • Skills/Certs: BLS • Other: Oncology clinic is attached to the chemo treatment center and the hospital, which will require providers to provide two years of inpatient case logs to qualify for privileges.

I have a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.

ADDITIONAL DETAILS If the provider has a clean file, we can pursue temporary privileges for a December start if not, we may be looking for more January. Providers with settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges.
