Locum Hematology/Oncology Physician - Outpatient Clinic, No Call, 2-3 Days per Week | NW Indiana
Job description
Start: November 1, 2026 (or as soon as credentialed)
Length: 13 weeks (open to extension)
Coverage: Outpatient clinic only - new patients and follow-up visits
Schedule: 2-3 days per week, 8:00-16:30 (Standard 5, 8-hour days)
Call: No call
Volume: Approximately 20 patients per day
Practice Setting: Acute care hospital-affiliated outpatient clinic
Practice Structure: 2 physicians, 2 APPs, 3 RNs, 2 MAs
New patient consultations and follow-up visits
Development and management of chemotherapy treatment plans
Application of NCCN guidelines in treatment decision-making
Management of emergency responses related to medication side effects
Timely documentation in Epic
Requirements
Board Certified in BOTH Hematology and Oncology
Active Indiana medical license required - in hand at time of submission
Active DEA required, or one easily transferable
NPI number required with submission
Epic experience preferred; training available for providers without Epic experience
Local candidates within 50 miles of the assignment are not eligible
Credentialing timeline: minimum 90 days
Travel & Lodging
Rental car (standard class), parking, mileage over 50 miles at IRS rate, lodging up to $183/night, flights up to $800, rideshare and tolls per policy
