A new study in the Annals of Internal Medicine concludes that hospital-based care transition programs may have better outcomes than community-based transition programs. The study reviewed outcomes for stroke patients who received transitional care in a variety of programs. Study authors caution that further study is needed of alternative transitional care programs.
