Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant.
Lead Nurse Practitioners and/or Physicians Assistants
Coordinate discharge for acute, short-term, and long-term placement
No hospital rounds – focus on comprehensive care, seeing an average of 8 patients a day.

Successful completion of a residency program.
Board Certification preferred
Active state physician license without restrictions
State CDS certification and DEA registration
Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
Seeking Primary Care Physicians in Internal Medicine, Family Medicine, Geriatrics or Emergency Medicine.
Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.

Very Competitive compensation DOE and Quarterly Bonus.
