## 🩺 JOB OVERVIEW

A community health practice is seeking a **Board-Certified Family Medicine Physician** to provide comprehensive outpatient primary care to an underserved patient population.

The physician will care for **pediatric and adult patients**, with an emphasis on preventive care, wellness, chronic disease management, acute care, and continuity of care.

### Key Responsibilities

* Provide primary care for pediatric, adolescent, adult, and geriatric patients
* Diagnose and manage acute and chronic conditions
* Conduct physical exams, preventive screenings, and wellness visits
* Provide pediatric well-child care, immunizations, and treatment of common illnesses
* Develop individualized treatment plans
* Educate patients and families on prevention and treatment
* Coordinate referrals and multidisciplinary care
* Maintain accurate documentation in Epic
* Participate in limited phone/beeper call
* Support quality improvement and patient-outcome initiatives
* Provide culturally sensitive, patient-centered care

## ✅ QUALIFICATIONS & REQUIREMENTS

* **Board Certified in Family Medicine – Required**
* Active state medical license at submission – Required
* Active DEA – Required
* Minimum **1 year of physician experience – Required**
* Strong outpatient primary care experience
* Comfortable treating both pediatric and adult patients
* Experience with underserved/community populations preferred
* Epic experience preferred
* Excellent clinical, communication, and decision-making skills
* Strong commitment to preventive and patient-centered care
* At least one professional reference from within the past 24 months
* Clean malpractice and licensing history highly preferred
* **No visa sponsorship available**
* Candidates requiring new state licensure sponsorship will not be considered

## 💰 COMPENSATION & BENEFITS

**Annual Salary: $287,000–$347,000**, depending on experience.
