Requirements:
• Provider type: MD/DO
• Board Certifications Req’d: Cardiology
• Fellowship Req’d: Yes
• State License(s) Req’d: Wisconsin and IMLC accepted
• Certifications Req’d: ACLS and BLS
• DEA Req’d: Yes
• Minimum Experience Req’d (# of years): 5
