· Trauma Level: Level II
Practice Setting:
• Inpatient
Requirements:
· Board Certification: Required
· Certifications required: PALS

Credentialing Timeframe: 60 days (temporary privileges available)
Active AZ License Required
Medical malpractice insurance, travel, and lodging provided
