This role focuses on evaluating, diagnosing, and managing patients with kidney-related conditions such as chronic kidney disease, hypertension, and electrolyte disorders in an outpatient collaborative setting.

✔️ Conduct comprehensive renal-focused patient assessments
✔️ Diagnose and manage chronic kidney disease, hypertension, and renal complications
✔️ Order and interpret labs including creatinine, BUN, GFR, and urinalysis
✔️ Develop and adjust individualized treatment plans with supervising nephrologist
✔️ Prescribe medications including antihypertensives and renal therapies
✔️ Educate patients on diet, medication adherence, and disease management
✔️ Coordinate care with dialysis centers and transplant teams
✔️ Monitor patient progress through follow-ups and documentation
