Nov 21-Dec 8 and Dec 27-Jan 5; Shift Times: 8:30am-5pm , 5pm-8:30am

Schedule Type: Clinic and Call

Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call

Provider will be expected to be in clinic and on call from 830am-5pm on weekdays (this will be paid at the regular hourly rate, call responsibility during the day is included in that regular rate paid). They will then be on night call from 5pm-8:30am the next day (paid at the night call rate). Weekends will be 24hr call coverage. We would like to note they may not be in clinic all 5 days during the dates listed but it is a possibility.

Requirements
· Board Certification: Must be Board Certified or truly Board Eligible within 3-5 years of residency is an option but we are looking at BC.
· License required: Active or IMLC
· Minimum experience required: 2 years
· DEA: Required
· Certifications preferred: FLURO - can accept temp FLURO as well
· Certifications required: Board certified

Credentialing Timeframe: 90 days
