Start Date: 7/17
End Date: 7/27
Coverage Type: Scheduled + Call
Schedule: Monday, Thursday, and Friday from 8:00am-4:00pm; Inpatient / ED after hour coverage is from 4:01pm-7:59am

Work Details:
· Patients per shift: Office 35 - 45/week individual visits Inpatient 2 - will confirm if Locum will be required to do surgery
· EMR: Prefer someone with EPIC experience but able to train if needed

Practice Setting:
• Inpatient

Requirements:
· Board Certification: Required
· License required: Must have ACTIVE IN license
· DEA: ACTIVE DEA (or one that is easily transferrable)
· CSR: ACTIVE CSR (or one that is easily transferrable)

Credentialing Timeframe: 60-90 days (ASAP)
