Requirements
Providers must have recent stroke care and be up to date on extended-window thrombolytic. Neurology residency training and Neurology board certification is required
· Board Certification: BC Only
· License required: Active WA or IMLC
· DEA: Required
· Fellowship: Required

Credentialing Timeframe: 90 Days
