Start Date: As soon as credentialed
Contract Length: Flexible
Schedule: PRN
Job Details: Pre-employment physicals for variety of jobs and clients, Injury care (initial/follow up/referral)
- Facility Type: Employee Health Clinic
- Practice Setting: Outpatient

Indiana License Required (We can help license you!)
Credentialing Timeframe: 30 - 60 days
Medical malpractice insurance, travel, and lodging provided
