Great opportunity for a provider comfortable with orthodontic cases to support two locations with consistent weekly scheduling.

📅 Assignment Details
* Dates: May 4 – July 31, 2026
* Schedule:
* Mon: 8:00 AM – 5:30 PM
* Tues: 7:30 AM – 4:00 PM (Green Bay)
* Wed: 7:30 AM – 4:30 PM
* Thurs: 8:00 AM – 5:30 PM (Appleton)

🏥 Practice Highlights
* 5 support staff
* EMR: Dentrix
* Adults + some cooperative pediatric patients
* Split between Green Bay & Appleton locations

🔍 Scope
General dentistry with strong orthodontic focus. Must be comfortable taking over existing ortho/Invisalign cases. Bread & butter dentistry included; complex cases can be referred out.

📋 Requirements
* Active state license (required)
* BLS (required)
* DEA (preferred)
* Invisalign & Nitrous certifications (preferred)

⏱️ Credentialing
* ~1 week turnaround
* New QGold TB test required within 3 days of offer (coordinated by team)
