LOCATION: Eureka, CA
SETTING: Scheduled Clinic and Hospital Call

ADDITONAL Requirement INFO:
• Must have California License
• No allergy testing or immunotherapy required.
**presented candidates must be available for a minimum of 4 consecutive months and 2 consecutive weeks per month.**

EMR Clinic = AllScripts
EMR Hospital = MediTech

Clinic Schedule: Monday - Friday 8:30am - 4pm
Call Schedule: 2 consecutive weeks per month - Call Rotation: 1:2 call

Caseload:
60% Adult
40% Pediatric
95% Office/Clinic
5% Hospital
60% Non-Surgical
40% Surgical

• Avg # of patients per physician per day: 17 with 16 of those being scheduled
• 1 add on's
• 6/week Surgeries
• Avg # of Hospital patients per physician per day: 0-2 Surgeries: 0-1
• Supervision - NP 4 days a week
Worksite Resources:
7 Exam Rooms
One MA per provider
Office Equipment: Nasal endoscopy, flex laryngoscopy, audio booth

Hospital Privs:
60 Days

• ASAP
• Ongoing
• BLS, ACLS, PALS/APLS - are required
