Job description

• Shift is Monday and Tuesday, but would love to grow the practice to another day or two per week, if the provider is willing and able

• Avg. Daily Census: 20-30 per Week
• Patient Age Group: Adults
• Procedures: Chronic Pain / Pain Management

Requirements:
• State License(s): MO & OK - Will license for Oklahoma; MO required in hand to present
• Certifications: BLS
• Years of Experience Req’d: 2+ Years
• Credentialing Timeframe: 1-2 Weeks
