Qualifications

ABMS/AOA board certified or board eligible in Physical Medicine & Rehabilitation (PM&R) or Anesthesiology.
Fellowship in Pain Medicine
Unrestricted licensed to practice medicine in New York or willing to license.
Active and unrestricted DEA registration
