• Practice Setting: Outpatient clinic. The clinical requirements would be only E&M visits, and we have a team of CDEs to provide diabetes education to our patients. No procedures or any other services would be needed.
• EMR/Charting Software: epic
• Avg. Daily Census: 12-15
• Patient Age Group: peds
