Part time Physical/Behavioral Health APP need in Fort Collins, CO.

Shift Details: 8-hour shifts, 8a-5p

Schedule: Monday and Friday, weekly

Work Details:
· Support staff: One MA and one RN. A therapist and case manager are also available to help with referrals, therapy and other client needs

· Amount of coverage needed per month: 2 shifts weekly

· Patients per day: ~12 pts per day

· Patient population/age: Clinic located within a homeless shelter - Co-occurring behavioral health and physical health needs

· Common Clinical Focus: Foot care, wound care, Infectious Disease
· Skills Required: Needs to be able to do physical medicine, some wound management, manage behavioral health and substance use (sometime active intoxication)

Practice Setting:
· Clinic inside of a homeless shelter

Requirements:
· Board Certification: FNP or PA preferred.
