Physical/Behavioral Health NP/PA Needed in Fort Collins, CO:
Start Date: Feb 2026
Contract Length: 3 months ongoing
Schedule: Mondays and Fridays: 8a-5p
Work Details:
· Patients per day: ~12 pts per day
· Patient population/age: Clinic located within a homeless shelter - Co-occurring behavioral health and physical health needs
· Common Clinical Focus: Foot care, wound care, Infectious Disease
· Skills Required: Needs to be able to do physical medicine, some wound management, manage behavioral health and substance use (sometime active intoxication)
Practice Setting:
· Clinic inside of a homeless shelter
Requirements:
· Board Certification: FNP preferred, PMHNP considered only if they are very comfortable with physical health and feel it’s firmly within their scope
Travel: Looking for a local, drive in candidate since the position is part time Mondays and Fridays

Credentialing Timeframe: Outpatient, very fast credentialing!
Active CO License Required
Medical malpractice insurance provided
