Schedule: Mostly 7am Friday-7am Monday call. This may vary with provider time off.

Requirements:
• Board Certification: board certified or eligible is fine
• License required: PA
• DEA: PA
• Minimum experience required: 5 years
Credentialing Timeframe: less than 90 days
