• Contract Length: 1 week (vacation coverage)

Schedule and Coverage
• Coverage Dates:
o May 26 (5:00 PM) – June 2 (7:00 AM)
o Provider to arrive during the day on May 26 for orientation
• Shift Structure:
o Inpatient rounding from 7:00 AM until completed
o Night call included
• Coverage Frequency: One time coverage only

Provider Requirements
• Board Certification: BC or BE (within 5 years)
• Licensure: Active state license required (facility not willing to license)
• DEA: Required
• Certifications: BLS, ACLS
• Experience: 1–2 years preferred

Credentialing
• Temporary privileges: Available in approximately 2 weeks
• Full privileges: 60–90 days
