• Provider type: MD/DO
• Board Certifications Req’d: Yes
• Fellowship Req’d: Yes or IMLC
• State License(s) Req’d: Wisconsin
• Certifications Req’d?: ACLS and BLS
• Specific Skills Req’d: Comfort with aortic dissections, thoracic surgery if needed
• CSR Req’d:
