February 18 (7:00 AM) – February 20 (7:00 AM)

Practice Details

Hospital Bed Size: 269 beds

Patient Population: Adult

Support Staff: Advanced Practice Provider (APP) during daytime hours

EMR: Cerner

Requirements

Board Status: Board Certified or Board Eligible in Urology

Licensure: Active Arizona license or IMLC Letter of Qualification required to submit

DEA: Required

Experience: Minimum of 1 year

Credentialing

Emergency credentialing available
