· Hospital Bed size: 269
Practice Setting:
• Hospital
Requirements:
· Board Certification: Board Certified or Board Eligible in Urology
· License Required: Active AZ license or IMLC LOQ required to submit
· DEA: required
· Minimum Experience Required: 1 year
Credentialing Timeframe: emergency offered
