Contract Details

• Practice Setting: Hospital
• Start Date: As soon as credentialed
• Shift/Schedule Details: 24 hour call
• Credentialing Timeframe: 90 days
• General Requirements:
○ Board Certification: BC or BE in Vascular Surgery
○ License Required: PA
○ DEA: Yes
• Patients per day: 10-15 consults per week, 6-10 surg per week
