Coverage Type: Scheduled Clinical Hours Only
Schedule: Provider will work 2 days per week in the outpatient surgery center next door and 3 days per week in the
clinic setting. We do prefer candidates that can give full time availability
Work Details:
· Support staff: Yes
· Patients per shift: 12-15
· Providers Requested: 1
· EMR: Epic
· Special procedures: All epidurals, facet injections, ablations, and other procedures are performed at the surgery center
Practice Setting:
• Outpatient
Requirements:
· Board Certification: Yes
· License required: Active CA License
· Certifications required: ACLS, Active FLURO is required. We will look at candidates that have the temp fluro in hand
Credentialing Timeframe: 60-90 Days

• 13 week with possibility of extending
• No on call
• All epidurals, facet injections, ablations, and other procedures are performed at the surgery center
